The chaos begins when Theo is rushed to Northern Districts Hospital after being trapped beneath a car in a shocking incident tied to the notorious River Boys gang.
His injuries are severe, and the situation quickly turns critical. As paramedics race him into the emergency department, doctor Bree arrives just in time to take charge of the case, working alongside Levi to stabilize the young mechanic before it’s too late.
Cash soon arrives at the hospital, clearly desperate for answers. However, emotions quickly explode when Sunny confronts him, furious that Cash ignored his earlier calls for help. Sunny insists the situation could have been prevented if Cash had responded sooner. The heated confrontation only adds to the chaos until Dana intervenes, urging Sunny to let medical staff examine his own injuries after he was beaten outside during the attack.
Despite being battered and shaken, Sunny struggles to recall exactly what happened. He reveals that he was dragged outside and assaulted by members of the gang before he could witness the moment the car was dropped onto Theo. However, he does remember hearing one of the attackers called by the nickname “Spanner,” a detail that could prove vital to the investigation.
Meanwhile, Theo’s condition deteriorates rapidly. Bree and Levi discover a dangerous build-up of fluid around his heart, a life-threatening complication that requires an emergency procedure known as a pericardiocentesis. Working against the clock, the doctors perform the delicate intervention, successfully relieving the pressure around Theo’s heart. For a moment, it appears the worst may have passed.
But the relief is short-lived.
Justin and Leah arrive at the hospital after receiving the devastating call, and they are horrified to see Theo fighting for his life in intensive care. Levi carefully explains the severity of the injuries and warns that although Theo has stabilized for now, further complications could arise at any moment. Emergency surgery may still be necessary.

While Theo fights for survival, Cash continues his investigation into the attack. Determined to bring those responsible to justice, he visits the River Boys’ hangout and questions several members about “Spanner.” However, the gang members remain tight-lipped, refusing to cooperate.
Unbeknownst to Cash, the very man he is searching for — Edmund — is standing right in front of him.
Realizing he may need another approach, Cash decides to question Lacey, believing she may know more than she is willing to admit. He invites her to the police station for a conversation, hoping she might help expose Gage and the rest of the gang.
Lacey appears calm and composed during the interview, but Gage is far less comfortable with the situation. Suspicious and protective, he insists on accompanying her to the station alongside Edmund and another associate, clearly unwilling to let her speak alone with police.
Back at the hospital, tensions escalate further when Justin confronts Sunny in a burst of rage. Dana has already noticed that Sunny has been hiding the severity of his injuries, but when Justin aggressively shoves him during their argument, Sunny collapses to the floor in agony.
Doctors quickly step in and discover extensive bruising across his ribs, raising fears of internal bleeding. Despite his condition, Sunny refuses to focus on himself, insisting that the only thing that matters is Theo’s survival.
Eventually, Bree convinces him to undergo a CT scan to ensure there are no life-threatening injuries.
